The document has a separate category for serving and retired members of the armed forces, paramilitary forces and police. Only one thing is added to the seven common requirements: the prescribed defence form.
One practical point: the form is hosted on IVAC's website, not the High Commission's. The High Commission document gives its address on `ivacbd.com`. Web links change over time, so if the form is not at that address, ask at the application centre; we are not reproducing the exact link here, because a broken link would send you the wrong way.
Note that this category is defined by profession, not by purpose of travel. Whether you are going as a tourist or for treatment, the form applies if you serve in these forces.
